| Back To School Packet - Download Form This form encompasses a wide variety of questions districts have their parents fill out before the start of the next school year. | Next Year Fall Sports Participation Form - Download Form This form restricts the user from selecting Sports only out of the list of Next Year Fall Season Sports. Adding this form requires the district to setup next year sports, which involves copying the Sports, Sports Seasons, and Athletic Releases to the following school year. When Athletics forms are filled out for students, new Sports Participation and Athletics Release records will be created on their Athletics screen in Genesis. Created by: Greg C. | Clubs Participation & Registration - Download Form Students will select one or more school clubs from the District>Activities list that they wish to participate in during the CURRENT school year. Selected club(s) will directly populate student Clubs tab for the current school year only. NOTE: You must indicate which clubs are available to sign up for and which clubs require 'approval' on this Genesis screen: School Clubs > Setup > Clubs Clubs that require approval will appear on the form with "Requires Administrative Approval" and students that signup will appear with the status of "Requested" until they are set to "Approved" using the School Clubs > Students > Approve Club Requests screen. It is also possible for the club to show with "This club is currently full" based on the maximum set on the School Clubs > Setup > Club Settings > Other Club Settings screen. | Nurses Required Health Information - Download Form This Form shows examples of medical information that can be collected within the Parent Forms. | Sample Contact Verification Form - Download Form This form showcases a special response type for contact verification, called "Confirm Contacts on Condensed Contact Screen". This redirects the parent to their 'Contacts' page in the parent portal, where they must review the contact information and click "Confirm" once everything is correct. Note: The district must have the "Enable Compact Contacts screen" setting enabled, on the Parent Access > Setup > Settings > School Settings > Contacts screen, in order to utilize this form. This new setting enables the parents to edit contact information one time, as opposed to making the same corrections to each child on the parent's account. Created by: Greg C. | FAFSA Waiver/Completed Survey - Download Form This was created as a starting point to your FAFSA Graduation Requirement form. Used to collect FAFSA requirement status of COMPLETED or WAIVED and will populate 3 fields on the student Graduation tab: - FAFSA Requirement: (Completed, Waived) - Completed Requirement Signed On (date form submitted) - Completed Requirement Signed By (name of parent/student account) Note: Parents cannot answer "YES" to both Completed and Waived questions. Responding "No" to both questions will result in no updates to the student's Graduation Note: This form was designed to be collected from Parents or Students only. | Student Vehicle Registration Form - Download Form This form is used to collect vehicle information from students for purposes of parking lot, parking permit, etc. Data from this form is used to populate student records instantly with updated Vehicle information. NOTE: Submitting this Form will cause existing Vehicle information to be overwritten.
