Setup for the Health History Update Form
Procedure
- Create a Document Management DocType for the Health History Update Form.
- Setup Athletic Eligibility to include the Health History Update Form - in schools that will need it.
- If you are going to allow parents/guardians to upload the form from the Parent Module:
- Set the correct settings on the Parents->Setup->Settings->School Settings->Athletics.
- Setup genMessage alerts for the staff members who review/approve the uploaded forms.
- If you will be using the Parent Form to collect Health History Update Form responses, and the ReportWriter report to generate the PDF forms for Student Documents:
- Download the Parent Form
- Download the ReportWriter Report
- Install the Parent Form
- Install the ReportWriter Report
- "Localize" the Parent Form & ReportWriter report so that they are linked together in your system
- Setup genMessage alerts for the staff members who review/approve the uploaded forms. (If you did not do this in Step 3 above).
Create a Document Management HHUF DocType
Before uploading Health History Update Forms (HHUF) into Genesis, it is necessary to create a Document Management "HHUF" DocType that can handle these appropriately.

Setup Athletic Eligibility to Include Health History Forms
Each school individually specifies what is required for 'athletic eligibility' to participate in a team sport. The allow the Health History Update Forms to be uploaded from the parent module, they must be required for eligibility in a student's school.

Enable HHUF Upload from the Parent Module
This is done on the Parents->Setup->Settings->School Settings->Athletics screen. Detailed information about this screen and how to enable the upload can be found in the following article: Athletic Settings.
Install the Parent Form/ReportWriter HHUF Pair

Enable GenMessaging for the HHUF Reviewers
It would be best if Health History Update Form reviewers receive "Document Upload" GenMessages whenever a Health History Update Form is added to a student's record.
This allows reviewers to be alerted whenever - and however - a Health History Update Form is uploaded to a student's record.
To do this:
- Reviewers must have access to the Document Upload genMessage (i.e. they must be able to subscribe to it)
- Subscriptions can be pushed out to the reviewers, if desired.
- The Subscriptions must specify that alerts should occur for documents of the "Health History Update Form" DocType - whatever you have named that DocType on your system (there is no pre-set doctype).
System Health History Update Form Rules & Responses
For the "Upload Health History Form" Button to Appear in the Parents Module
- There must be a Health History Update Form DocType in Document Management
- Athletic Eligibility must include "Health History Update Form" for the students' schools.
- The student must already have a "participation" record for a sport where the season is upcoming. (Note that the Parent Form for the Health History Update Form will itself create this record).
- The student must have a valid (i.e. unexpired) physical examination on file, but older than 90 days.
- It is currently within 90 days of the scheduled season start for the upcoming sport season.
For a ReportWriter Report to Extract Data from the Parent Form
The Parent Form is identified by a "Form ID" on your system. Every Parent Form has a unique Form ID. If you copy a Parent Form - or upload one - it will receive a new, unique Form ID.
A ReportWriter report, to extract data from a Parent Form, must be able to reference it's unique Form ID. The uploaded ReportWriter report will need to be "localized" to 'know' the Parent Form's Form ID. See the detailed instructions in the following article: NJ DoE Athletics Health History Update Form.
