The History Area of the A-45

This section of the A-45 is a pre-printed set of Diseases and Conditions with an associated "year" field for each:

There is one available blank field into which any additional condition or disease can be written.  

The "Year" field specifies the year the student contracted the disease or was diagnosed with the condition.  Blank year fields indicate the student not having or having had the condition or the disease.

Three screens are sources for populating the History area of the A-45:

Configuring Conditions to Appear on the A-45

Individual Conditions can be mapped to specific "History Record" items.  The History Record item (see below) is then mapped to the A-45.

This is done to allow a date / year to be entered for when the student had a specific condition that needs to print on the A-45.

The link between Conditions and History Records is defined on the Nurses→Setup→Codes→Conditions screen:

The "Tied to History Record" column is used to link an individual condition to its History Record.
