Tests and Therapy Screen

This screen captures information for four areas on the A45 form:

• TB Screening
• Chest X-Ray
• Lead Tests
• Therapy

The information captured here is exclusively for the A45.

Each of the four separate areas on this screen has its own dedicated area on the A45.   The items captured in the top row can all capture as many tests as necessary, though only the most recent will print on the A45.  The "Therapy" area at the bottom of the screen is fixed and cannot be extended.

TB Screening (Mantoux Test)

The information in the "TB Screening (Mantoux Test)" area of the Tests/Therapy screen prints in the TB Screening area of the A45:

There is only room for two TB Screenings (Mantoux or IGRA Test) on the A45.  

If there are more than two records in the "TB Screening (Mantoux Test)" area of the student's Test/Therapy screen, only the two most recent will be printed on the A45.

To capture a new TB Screening record, enter information on the "Add Screening" line at the bottom:

• Date the student was tested
• Date the test was read
• The Mantoux result
• The IGRA result.

Clicking the "Save Screen" button will generate a new record if any of the fields in the "Add Screening" area are filled in.

Any of the fields can be updated at any time.  To store the changes, similarly click the "Save Screen" button.

Chest X-Ray

The information captured in the "Chest X-Ray" area of the Tests/Therapy screen is printed in the "Chest -Ray" area of the A45:

There is room for four Chest X-Ray dates/results on the A45.  If there are more than four Chest X-Rays captured on the student's Tests/Therapy screen, only the four most recent X-Rays will print on the A45:

To capture a new Chest X-Ray record, enter information on the "Add X-Ray" line at the bottom:

• Date of the X-Ray
• Result

Clicking the "Save Screen" button will generate a new record if the Date field in the "Add X-Ray" area is filled in.

Any of the fields can be updated at any time.  To store the changes, similarly click the "Save Screen" button.

Lead Tests

The information in the "Lead Tests" fields populates the "Lead Test" area on the A45:

The A45 form only has room for the four Lead Screenings.  If there are more than four Lead Test records on the student's Tests/Therapy screen, the four most recent Lead Tests will be printed on the A45.

To capture a new Lead record, enter information on the "Add Lead Test" line at the bottom:

• Date of the test
• Result

Clicking the "Save Screen" button will generate a new record if the Date field in the "Add Lead Test" area is filled in.

Any of the fields can be updated at any time.  To store the changes, similarly click the "Save Screen" button.

Therapy

The information captured in the "Therapy" area at the bottom of the Tests/Therapy screen, prints in the "Medication Reactor" area at the bottom right corner of the first page of the A45 form:

No additional records can be added for the "Therapy" section.

You can update the four existing fields at any time:  enter information and click the Save Screen button.
