Disease and Injury A45 Medical History Screen

This screen is designed to capture information for the following areas of the A45 form:

  • Disease History
  • Operations or Injury
  • Serology Titers

The screen is divided into three columns:

  • Disease History - The red starred items print in the "History" sections of the A45

    Enter a date only if the student has had the corresponding disease or condition.  Only a single year can be captured for each of these fields - they are designed only to populate the form.
  • Operations or Injury - Information is captured but not printed on the A45 at the moment (no dedicated fields).  Enter a date only if the student has had the corresponding injury or condition.  
     These currently do not print on the A45 as there are no fields dedicated to them.
  • Serology Titers - These are designed exclusively to capture information for the corresponding areas on the A45. This information prints in the "Serology Titers" section of the A45:

Updating Disease/Injury and Serology Titer Fields

Update all desired fields and click the Save button at the top or bottom of this screen.

Adding Additional Diseases or Injuries

At the bottom of both the Disease Record column and the Injuries column, there are "Add" fields where new items can be added to either column:

To add a new entry, enter the name of item into the proper 'Add' field ("Add History" or "Add Injury"), then click the Save button at the bottom of the screen.  

The new field will now appear at the bottom of the appropriate column and you can fill in a year.

WARNING: When you add an entry to either the "Disease History" column or "Injury" column, you are not adding it for just the one student:  you are adding the new item to the entire system and it will appear for all students in all schools - for all students in the school district.
