Medical Notes

The student's medical Notes screen (Student Data>Modify Student>Medical>Notes) is populated from three places:

  • The nurse can directly create Notes on this screen.
  • All 'notes' or comments tied to Screenings are also listed here.
  • All 'notes' or comments for Immunizations are also listed here.


Notes on the Notes Screen

Individual Notes are identified by where they came from:

  • If there is a date in the Screening column, the Note originated as a Screening note for the screening done on the listed date.  The Note can be updated directly on the Notes screen or on the Screenings screen.
  • If there is an Immunization listed in the Immunization column, the Note originated as a note for the listed Immunization.  The Note can be updated directly on the Notes screen, or on the Immunization screen for the listed Immunization.
  • If neither an Immunization or a Screening is listed, the Note was entered directly on the Notes screen by a nurse.  It can only be updated here.

Determining whether or not a Note will print on the A45 Form:

The "For Medical Form" column contains a check mark next to every Note that will print on the A45.  If the "For Medical Form" column is blank for a Note, it will not print on the A45.  You can control whether individual Notes - from any source - print on the A45.  See below in the "Controlling Whether a Note Prints on the A45 Form" section.

Entering a New Note - Adding a Note

The nurse can add a Note directly on this screen by clicking the button.  

This brings up an "Add Note" dialog box:

The "Date" field defaults to 'today' but can be adjusted to any date.

The Text field accepts only standard, "ASCII" text - no bold, no italics, no graphics.

The "Show on Medical History Form" controls whether or not the Note will print on the A45 form.  

If the checkbox is not checked, the Note will not be printed on the A45.  

When the checkbox is checked, the Note does print on the A45.  

The checkbox can be edited later:  Notes can be included/excluded from the A45 at will.

When you are done, click Create to create the new Note or click Cancel to abandon your changes.

Deleting a Note

Any Note can be removed by the clicking the delete trashcan icon at the right side of its row.  This will delete the Note.  

If the Note is a Screening Note, it will also be removed from the Screening.  if the Note is an Immunization Note, it will also be removed from the Immunization.  

There is no UNDO action for the removal of Notes.

Editing a Note

To edit a Note, locate it on the Notes screen (), and click the  "edit icon" for the Note.  

This brings up its "Modify Note" dialog box:

You can alter the date of the Note, whether or not it prints on the A45 form (for non-Screening related Notes) and the text of the Note.

When you are done, click Save to store the changes.

Controlling Whether a Note Prints on the A45 Form

Immunization Notes, and Notes the nurse enters directly on the Notes screen can be excluded from the A45, or included on the form, via the "Show on Medical History Form" checkbox on the "Edit Note" dialog.  

Notes from Screenings always print on the A45 and this cannot be altered.

To change whether a Note should print - or not print - on the A45, click the

 "edit icon" for the Note.  This brings up its "Modify Note" dialog box;

To ensure the Note prints on the A45, make sure the "Show on Medical History Form" checkbox is checked.

To prevent the Note from printing on the A45, uncheck the "Show on Medical History Form" checkbox.

Click Save to store your changes.

Notes related to Screenings are required to print on the A45:
