Immunizations Screen

This is found via Student Data>Modify Student>Medical>Immunizations.

The Immunization Screen is designed primarily for capturing student's immunization information to be used for two purposes:

• Print Immunization information on the NJ A45 form
• Determine if students are in compliance with NJ Immunization requirements.

The screen can also be used to capture any information where you wish to record multiple dates for items.

Genesis configures this screen with the list of immunizations needed for printing information on the NJ A45 form and for determining if students are in compliance with NJ Immunization information.  

Your district can locally add items to the list of "Immunizations".   

Fields at the Top of the Immunizations Screen

The top area of the Immunizations screen contains four fields designed primarily to populate the corresponding fields on the NJ A45 form.  

These fields do not indicate that there is additional information stored elsewhere in Genesis:

• Medical Exemption Attached - The checkbox will populate the "Medical Exemption Attached" field on the A45. It does not automatically attach any documents to the A45 form however. It is possible to upload a "Medical Exemption" document into the student's record in Genesis if your district has the "Document Management" enabled, however these documents currently do not display with the student's A45 printout.
• Religious Exemption Attached - The checkbox will populate the "Religious Exemption Attached" field on the A45.  It does not automatically attach any documents to the A45 form however. It is possible to upload a "Religious Exemption" document into the student's record in Genesis if your district has the "Document Management" enabled, however these documents currently do not display with the student's A45 printout.
• Provisional Admission date - Printed in the "Provisional Admission Date" field on the A45.
• Immunization Registry # - This also appears on the student's Health Record screen.

On the A45, these fields appear immediately below the list of Immunizations and above the list of Diseases and Injuries:

Immunization Alert Detected Warning and Report

When this warning appears, it means Genesis has detected an issue with the student's compliance with NJ Immunization Requirements.

Click the link for a report explaining the detected issues.

If the warning icon does not appear, the student appears to be in compliance with NJ Immunization Requirements.

Immunization Records

Each row in the Immunizations screen captures the information for a single Immunization:

The first column, "Disease Date" should be populated ONLY if the student actually had the diseases (e.g. Mumps).    

It should be empty if the student never had the disease.

Columns labeled "Dose 1" to "Dose 6" capture the dates for up to six separate doses of this immunization/vaccine (etc).

The column labeled "Ex" should be checked if the student has been exempted from this immunization.  

Exemptions can be for the following reasons:

• Medical Exemption - student cannot receive this immunization for medical reasons
• Religious Exemption - student cannot receive this immunization because of Religious objections

Comments:  There is an "Add Comments" button available for each Immunization.  Comments entered here will by default print on the A45 form, though this can be controlled on the Notes screen.

Updating Immunizations

To update immunization information, update all fields you wish to update, then click the Save button either above or below the list of immunizations.
