Physical Exams, Release Forms, and Parent Forms
Physical Exams
Student Physical Exams are maintained in the "School Year" information on student's Athletics screen (Student Data>Modify Student>Athletics>Exams and Participation):
From this screen you can maintain a student's Physical Exam date and status, as well as a student's Steroid Release date and status, Concussion Release date and status, Cardiac Release Form date and status:
Automatic Expiration of Physical Exams: 365 days after the date listed in the Steroid Release, Concussion Release or Physical Exam date fields, the respective status will be switched from Completed to Expired.
Release Forms
Release Forms can be viewed/modified/created via Athletics>Setup>Athletic Release Forms.
Parent Forms
You can create forms on the Parent Access>Forms screen that tie back to Athletic data.
Response Types for Athletic Forms are:
- Steroid Form
- Concussion Form
- Cardiac Form
- Opioid Form
- Any Custom Forms you've created
Those options also have an additional field of "School Years to Display" which can be selected. When the user checks selected for this athletic release form, this field controls what schools years are available to be selected by the user.
Note: Make sure Athletic Forms are set up for next year. There is a tool on the linked screen that can copy this year's setup to next year. This is done via Athletics>Setup>Athletic Release Forms.
Response Types for Sports are:
- Sports Participation - Allows students to register for any sport in the current school year
- Fall Sports Participation - Register for a Fall season sport in the current school year
- Winter Sports Participation - Register for a Winter season sport in the current school year
- Spring Sports Participation - Register for a Spring season sport in the current school year
- Next Year Fall Sports Participation - Prior to Summer Rollover, students can register to participate in any Fall season sport for the NEXT school year only.
